How do you avoid this?

Flush over flush, didn't have a solid read on this guy, I figured him for playing position post flop.

If you figured him for playing position you didn't act like it. here's my advice:

1) you don't play 45s in early position
2) you raise on the flop to drive out drawing hands (this probably wouldn't have worked here as he had top pair too, but you'd have got info that he liked his hand)
3) you raise on the turn to see what he's representing. He would, I'm sure, have gone all-in or at least re-raised and you'd have been able to regretfully but sensibly lay down
4) you STOP CALLING. Try and discipline yourself even at these levels to call ONLY when playing tactically. Check, raise or fold 90% of the time and you will control games. Calling is weak and gets you no info on your opponents' hands, and by definition it means you're letting someone else call the shots. You want the hand to be played so it's ideal for YOU.

Think how much easier this would have been if you had position on him.
