o Not shifting gears against players who won't back down to your aggression
o Blowing people off worse hands, not inducing bluffs from people just itching to float you.
o Too much cheap street aggression building big pot with marginal holdings against opponents who otherwise suck too hard to win a big pot with a big hand.
o Running bad tilt, turning lots of little things into a very big thing

Variance is a big part of it. But I think mostly is that when you're winning consistently, you feel invincible and that you can beat anyone, and you don't really look at your game enough. You don't try improving, but everyone else is improving in the meantime.
Only when you're losing do you stop to think that something is wrong. Which is probably not the best way to go about thigns.
Games are definitely getting harder as well, or at least on my site.

By the mid stakes people get that, just like they get how position is good too, but they don't really get just how important it is.
When you're betting/raising on the cheap streets (usually pre-flop and the flop) a lot it has a couple effects that can pwn you on future streets. It tightens up our opponent's range and it builds the pot. So we tend to face stronger hands for bigger stakes. I think at some point a naked thin +EV bet becomes -EV because we will sometimes continue to play the hand with the worst of it for more money than if we checked behind.
Also, sometimes our aggression allows strong holdings to hide under the shadow of our aggression and still get good value without defining their hand as much as they would have to if we kept the pot a bit smaller or let our opponent to continue to define his own hand with (poorly applied) aggression.
Stuff to consider: Opening bet size, 3-bet size/frequency. Cold call vs 3-bet range, limp behind vs iso-raise range, c-bet frequency, etc. All spots where you can adjust your range to either exploit people who fold a lot on cheap streets or tighten up to have a stronger overall range when they don't.
