Default an unclear scenario about finish positions

Ok, I don't have HH here, but sg like this:

    Hero (UTG) 1200
    BU 1500
    SB 6k
    BB the rest

Hero has 66, hero shoves. BU Calls. SB calls both.
    BU has 33, SB AT.
A flops, SB wins, hero and BU are both out.

What determines which one of the players (BU/hero) finished 4th, which one 3rd?

This has happened me before as well, and I have no idea how it's determined.. so far seems to be rather random..

The amount of chips at the beginning of the hand determines the results, so in the example you gave the hero (1200) chips would be 4th and the button (1500 chips) would be third.

If both the Button and Hero are eliminated, the person with the most chips comes 3rd. In this case, you would come 4th since the button has more chips than you.

Default A Q fold for ITM

Yes, it is based on chip stack at the beginning of the hand.
I came in 4th once(out of the money) because I did not know this rule.
The next time this occurred with 4 remaining players at the local tavern, I noticed everyone went all in. I assume because they wanted to be sure I didn't win my blind. I happenened to fold with 115 chips remaining, and the chip leader who went all in took out both remaining players that had more chips than me , so I came in 2nd. I won by folding.

What happens when both 3rd and 4th have the same amount chips and get busted out at the same time?

I think that the player that is closest to the button gets the extra chip, so in this case would finish 3rd whilst the other unlucky person would finish 4th.

I think they split the prize of 3rd place

Could be, that would seem to be the fairest outcome
