In SB: Min-raising ATC or close to it seems good, at least until he adjusts. You can try to mix in some limps too, if he seems passive in the BB.
In BB: If he min-raises you every time then your situation is identical to when it's folded to you in the SB in 3+ handed table. Only difference is that there's no ICM factor. If he raises more than the minimum, then, obviously, you need to play a bit less liberally.

No. If you decide that, for example, 25% is the right 3-betting frequence, then you need to 3-bet whenever you get a hand that's in the top 25%. The exact sequence of cards is irrelevant.

There are people whose default strategy is to open every time, but when I adjust they usually re-adjust.
I also remember when Isildur1 played durrrr and Patrik Antonius, they had an agreement to open every time. It created some record-breaking pots and lots of publicity for FT.
